2. ELIGIBILITY. Life Uniform's "Win a Six Day Caribbean Cruise for Two!" Contest is open to legal residents of the continental United States and Washington D.C., 18 years of age and older, except the employees and their immediate family members (defined as spouses, parents, siblings and children) of Life Uniform or any of their vendors, suppliers, parent and related companies, and agencies. All federal, state and local rules and regulations apply. Void in the states of AK, HI and RI, and where prohibited or restricted by law.
3. HOW TO ENTER. An eligible participant must either: a) enter online at www.lifeuniform.com, click on "Win a Six Day Caribbean Cruise for Two Contest" and follow instructions; b) obtain an official entry form at any Life Uniform store, complete form and enter at the store; or c) obtain an official entry form at any Life Uniform store, complete form and mail to: Life Uniform Win a Six Day Caribbean Cruise for Two Contest, 2132 Kratky Road, St. Louis, MO 63114. Limit one entry per person per day. All entries become the property of Life Uniform and will not be returned.
4. PRIZES. One winner will receive a six day Caribbean cruise for two, including round trip coach air transportation for two, from the commercial airport closest to the winner's residence to the commercial airport closest to the location of the cruise departure. Cruise includes standard, double occupancy accommodations. All expenses, upgrades, shore excursions, or any other incidental or additional charges, not specifically provided for by Life Uniform in these rules are the sole responsibility of the winner. Travel arrangements must be made through Life Uniform and scheduled between March 1, 2008 and October 1, 2008. Winner and guest must travel together. Reservations based on availability. Blackout dates may apply. Other prizes to be awarded are five $100 Gift Cards to Life Uniform. The prizes are non-transferable and no cash or other substitutions are allowed, except by Life Uniform, which reserves the right to substitute a prize of equal or greater value. All applicable taxes on the prizes are the sole responsibility of the prize winners.
5. WINNERS. Winners will be selected at random from all eligible entrants on or about February 8, 2008. Approximate value of the Caribbean Cruise for two, including airfare, is $2,500. Actual cash value will vary, depending on flight itinerary and travel dates. The odds of winning depend on the number of eligible entries received. The winners will be notified via US mail within two weeks after being chosen and do not need to be present to win. Return of any prize notification as undeliverable will result in disqualification and selection of an alternate winner.
6. DURATION OF CONTEST. The contest begins on January 1, 2008 and ends on January 31, 2008. All entries, submitted on our website or at one of our stores, must be received by midnight EST on January 31, 2008. Mailed entries must be postmarked on or prior to January 31, 2008. Life Uniform assumes no responsibility for late or misdirected entries.
7. CONDITIONS OF PARTICIPATION. By participating, an entrant agrees to abide by and be bound by the official rules. Entrant agrees that in case he/she is a winner, his/her name, likeness, photos, city of residence, written words or voice can be used by Life Uniform for advertising, promotion or publishing. The prize winner will be required to execute an Affidavit of Eligibility and Liability/Publicity Release and return within ten business days of the date on the prize notification letter, otherwise prize will be forfeited and an alternate winner will be selected. By entering and/or by accepting a prize, you agree to be bound by these official rules and to release Sponsors from any and all liability arising out of your participation in this contest with respect to the awarding, receipt, use and/or misuse of any prize.
8. LIST OF PRIZE WINNERS. For a list of prize winners, mail a self-addressed stamped envelope to: Life Uniform, Attn: Contest Winner List, 2132 Kratky Road, St. Louis, MO 63114.
